HSE Observation

Objective: It allows user to create, review and approve/reject HSE Observation in the system. All the roles in the system can create HSE Observation.

Below steps to be followed to complete HSE Observation -

  1. Site Visit Completed Without Observations (Safe Act):
    • Report HSE Observation: To create new HSE Observation. Observation shall be done and reported by any system users. And it is closed after reported.
  2. Unsafe Act -> Safety Attitude Improvement Slip issued – Further investigation required:
    • Report HSE Observation: To create new HSE Observation. Observation shall be done and reported by any system users. If It is reported by Field Engineer then it is closed after reported.
    • Review HSE Observation: “Field Engineers” will review the observation once it’s reported.
  3. Unsafe Act -> Safety Attitude Improvement Slip not issued – Completed with verbal warning:
    • Report HSE Observation: To create new HSE Observation. Observation shall be done and reported by any system users. If It is reported by Field Engineer then it is closed after reported.
    • Review HSE Observation: “Field Engineers” will review the observation once it’s reported.
  4. Unsafe Act:
    • Report HSE Observation: To create new HSE Observation. Observation shall be done and reported by any system users. If It is reported by Field Engineer then it is go to Branch HSE Officer review.
    • Review HSE Observation: “Field Engineers” will review the observation once it’s reported.
    • Approve Observation: “Branch HSE Officer” will review and approve the observation.
  5. WOW Slip issued – Positive Safety Behaviour:
    • Report HSE Observation: To create new HSE Observation. Observation shall be done and reported by any system users. And it is closed after reported.
  6. New Job Commencement-Shaft Joint Inspection:
    • Report HSE Observation: To create new HSE Observation. Observation shall be done and reported by any system users. And it is closed after reported.
  7. Modernization Joint Inspection:
    • Report HSE Observation: To create new HSE Observation. Observation shall be done and reported by any system users. And it is closed after reported.

REPORT HSE OBSERVATION

1. Site Visit Completed Without Observations (Safe Act)

  1. Select the Branch, Department, Company Employee/Sub-Contractor Employee, While working with, Observation Type and Activity Status from drop down list which are mandatory to select.
  2. Enter "Job Number/ Project" which is mandatory to enter.
  3. If Company Employee/Sub-Contractor Employee is selected as Sub-Contractor Employee then select Sub-Contractor Compony from provided drop down list.
  4. Select “Date” and enter “Time”.
  5. Enter “Observation Detail” which is mandatory to enter. (Maximum 4000 characters allowed to enter)
  6. Click “Add Person Observed” button to add injured person who are injured in an incident.
  7. In “AGMelco Employee/Sub-Contractor Employee” area select “Employee(s)/Sub-Contractor(s)” from drop down list.
  8. Enter “Contractor” if “Employee(s)/Contractor(s)” is selected as contractor. (Contractor name will be displayed automatically when first 3 characters are typed)
  9. Enter Employee Code/Gate Pass, Company, Employee Name. (All are mandatory to enter)
  10. Click Save button to save the entry and Delete button to delete the entry.
  11. After save data click Edit button the make changes in the entry details.
  12. Click Attach button to select file as attachment.
  13. Click Upload button to upload the attached file and click Delete button to delete the file. (Each attachment files size upload limit up to 5MB and number of file attachment allow up to 20. Attachment files extension would be allowed such as “.JPEG, .PNG, .BMP, .XLS, .XLSX, .DOC, .DOCX, .RTF, .PDF, .MP3, .MP4 and .M4A”)
  14. In-case of multiple attachments click “Upload All” button to upload all attached files/images at a time.
  15. Click “Submit” button to save the entered data. HSE Observation will be close after Submit.
  16. Click “Save As Draft” button to save the entered data without send for review.

  18. After save data click Edit button the make changes in the entry details and Click Attach button to select file as Before attachments.
  19. Click “Submit” button to save the entered data. HSE Observation will be close after Submit if it is reported by Field Engineer otherwise it will go for Field Engineer review.
  20. Click “Save As Draft” button to save the entered data without send for review.

5. WOW Slip issued – Positive Safety Behaviour

  2. Enter "Job Number/ Project" which is mandatory to enter.
  3. If Company Employee/Sub-Contractor Employee is selected as Sub-Contractor Employee then select Sub-Contractor Compony from provided drop down list.
  4. Select “Date” and enter “Time”.
  5. Enter “Observation Detail” which is mandatory to enter. (Maximum 4000 characters allowed to enter)
  6. Enter "No of Persons" which is mandatory to enter.
  7. Click “Add Person Observed” button to add injured person who are injured in an incident.
  8. In “AGMelco Employee/Sub-Contractor Employee” area select “Employee(s)/Sub-Contractor(s)” from drop down list.
  9. Enter “Contractor” if “Employee(s)/Contractor(s)” is selected as contractor. (Contractor name will be displayed automatically when first 3 characters are typed)
  10. Enter Employee Code/Gate Pass, Company, Employee Name. (All are mandatory to enter)
  11. Click Save button to save the entry and Delete button to delete the entry.
