REVIEW INCIDENT INVESTIGATION (Investigation team member)

  1. Login as investigation team and go to My Actions screen (My Actions will be displayed by default after login)(Image 1).
  2. All the incident details will be populated in a tabular manner.
  3. There are four section provided to view and take action against tasks which are mentioned below :
    • FOR ACTIONS : Display “Incident”, “HSE Observation”, “Near miss”, “Safety Patrol” and “Management Safety Patrol” tasks against which action to be taken. Also display the tasks which are saved as draft are displayed here. user can edit and make complete a draft task. (Display by default)
    • FOR REVIEW : Display the tasks such as “Incident”, “HSE Observation”, “Near miss”, “Safety Patrol” and “Management Safety Patrol” whose review is pending. This tasks will be displayed to reviewer for review where he/she can take action against it.
    • FOR INFORMATION : Display all the tasks such as “Incident”, “HSE Observation”, “Near miss”, “Safety Patrol” and “Management Safety Patrol” to creator/requester with their current status. User can view the task details by clicking subject in “Subject” column.
    • UPCOMING TASKS: Display all the tasks such as “Incident”, “HSE Observation”, “Near miss”, “Safety Patrol” and “Management Safety Patrol” which assign to reviewer for upcoming days. User can view the task details by clicking subject in “Subject” column.
  4. When “FIR No - Send for Investigation” is clicked “Report Incident” screen will be opened. (Image 2)

  5. Click FIR Detail tab to view the FIR details which will be in read only mode.
  6. Click Investigation tab to view the details. (Visible by default)
  7. Click the sections to expand them and user can change and add incident details in it. (Optional)
  8. Click “Section A : Incident’s Basic Details” to add/edit basic details (Image Section-A)

  9. User can add/edit Type of Incident, Sub Category, Event Date, Event Time, Damage to Asset/Property.
  10. Enter Exact Location, Equipment Involved, Material Involved and Description of what happened in the text area provided. (Description of what happened is mandatory to enter)
  11. Enter “Immediate Action Taken (Also mention who visited site first and what action was proposed by him? if applicable)” in the text area provided.
  12. Click “Section B : Investigation” to add/edit investigation details (Image Section-B).

  13. Enter Description of how incident happened, Sequential occurrence, Activity of the person(s) /injured at the time of the incident, Observations and inferences (Plant evidences, operating practices, injured person experience and relevant training for the work performed etc. which may be cause indicative or helpful to conclude sequence occurrence/cause of the incident) in the text area provided.
  14. Select “If similar incident reported earlier?” as Yes/ No. If yes then provide Date of similar Incident.
  15. Click “Section C : Injured Person Details” to add/edit injured person details (Image Section-C).

  16. Click Add button to add injured person.
  17. Select “Incident Type” from drop down list.
  18. Enter required details in “Personal Details” section.
  19. In “Injury Details” section select “Nature of Injury”, select “Body Part”.
  20. Enter Injuries to internal organ, Activity at the time of Injury, Brief about Medical Treatment, Treatment given by in the text area provided.
  21. Click “Section D: Statements” to add/edit employee details (Image Section-D).

  22. Click “Add Statement” button to add employee and their statement of an injured person/Informer/witness.
  23. Select Category as Injured person/Inform By/witness person from drop down list.
  24. Select name from drop down list.
  25. Enter Position, Contact and Statement.
  26. Click attachment button to attach images or documents.
  27. Click Save button to save the details and cancel button to cancel the details and delete button to delete a particular entry.
  28. Click “Section E: Incident Cause” to add/edit Incident Cause (Image Section-E).

  29. In “Root Causes” section select below details.
    • A) UNSAFE ACTS
      1. Select Tools or Equipment.
      2. Select Noncompliance to proper procedures.
      3. Select Protective Methods.
      4. Select Distraction / Inattention.
    • B) UNSAFE CONDITIONS
      1. Select Protective Systems.
      2. Select Tools, Equipment & Vehicles.
      3. Select Exposures to.
  30. Select “Direct Causes” details.
  31. Select Work Place Environment.
  32. Incident Causes if selected, then CAPA task will be auto-generated that the user can however edit/delete.
  33. Click “Section F : IMPROVE AND CONTROL(Recommendations)(Image Section-F).

  34. Add “IMPROVE & CONTROL (Corrective & Preventive actions) - Direct Causes”
  35. Select Observation. Enter Action to be Taken, Select Type, enter Responsible Person, select Priority, select Target Date.
  36. Click “Cost Calculation of Incident” to add/edit Incident cost (Image Section-Cost Calculation).

  37. Enter Status of Direct Cost and Status of Indirect Cost.
  38. Enter Head of Cost in each section and attach images or document if required.
  39. Click Attach button to attach images or documents.
  40. Click Attach button to attach images. (Total attachment files size upload limit up to 20MB and number of file attachment allow up to 10. Attachment files extension would be allowed such as “.JPEG, .PNG, .BMP, .XLS, .XLSX, .DOC, .DOCX, .RTF and .PDF”)
  41. Click Upload button to upload the attached image and click Delete button to delete the image.
  42. In-case of multiple attachment click “Upload All” button to upload all attached files/images at a time.
  43. Enter comments which is mandatory to enter if “Next Action” is selected as “Investigation Completed & Send for Branch HSE Head Review”. (If Next Action is selected and Save button is clicked task will be sent for review. Is Next Action is not selected and Save button is clicked then details will be saved).
  44. Select “Review Person”. The members available will be populated automatically in the list. (Only Branch HSE Head members can be added)
  45. Click “Submit” button to save the details.

Note :

  • Reviewer can edit the report contents.
  • The fields marked with * are mandatory to be filled.
